What information do we collect?
As a minimum, we store your email address, emergency name and emergency contact number. We have identified key pieces of information that will help the 999 operators handle your emergency quicker.
Your medical information (allergies, medication, etc) and NHS number are encrypted in our database. You can delete your profile data at any time.
The email address you provide will be used to send you information, respond to inquiries, and/or other requests or questions.
SURVIVA Ltd. is the currently the provider of this service. When purchasing of new SURVIVA Scan stickers through our on line store, appropriate details will need to be collected to complete that transaction.
We do not store or collect credit card information on our servers.

We implement a variety of security measures to maintain the safety of your personal information when you enter, submit, or access your personal information.

SURVIVA Scan subscription information
Once activated, the SURVIVA Scan profile will be viewable online for two years. After which time a £1.00 GBP per year will be applicable to keep the profile live. Reminder emails will be sent out by SURVIVA Scan 3-4 times a year to the registered email address in order to keep details as up to date as possible. SURVIVA Ltd. has the right to increase the subscription cost in line with inflation should it see fit. After two years the profile will be stored offline and the stickers will cease to link directly to the profile but the profile information will not be deleted unless deleted by the user for a period of 5 years. The user will not be charged a subscription if their profile is not active.
After 5 years of inactivity profiles will be completely removed from the system.
